Death Certificates

The Boulder County Coroner's Office and the mortuary designated by the family will complete the death certificate. The completed death certificate is then filed with the Boulder County Public Health Vital Records Section.  Public Health has offices in Boulder, Longmont or Lafayette. For additional information, please call 303-441-1158. Certified death certificates are issued by the Heath Department, not by the Coroner's Office. Death certificates are not a public record. Individuals demonstrating a tangible interest may obtain copies of certified death certificates from the Health Department or through the mortuary that the family has designated.
