How to complete a paper voter registration form

The easiest way to register to vote in Colorado is online at www.GoVoteColorado.gov. To register online, you will need a Colorado Driver’s License, Colorado State ID, or the last 4 digits of your Social Security number*. If you do not have one of these, you can complete this paper form and return it to the Boulder County Elections office via email, mail, or fax.

You can type all or your responses into the PDF (except for your signature – line 9) or you can fill everything out by hand.

*If you register online using the last 4 digits of your SSN, you will be asked to complete additional steps to provide us with your signature.

Required Sections (marked with an * on the form)

  • Line 1: Confirm you are a U.S. citizen
  • Line 2: First & Last Name (legal)
  • Line 3: Birthdate + Colorado Driver’s License/State ID number or birthdate + last four digits of your Social Security number.*
  • Line 4: Address where you reside. If you do not have a fixed permanent address, you can still register to vote. See the Secretary of State’s Homeless Voters FAQs for instructions (you will need to list a “home base” as your residential address and also provide a valid mailing address – Line 5).
  • Line 9: Read the self-affirmation then sign and date.

*If you do not provide a CO driver’s license or ID number, you will need to provide documentation of Acceptable ID the first time you vote.

Optional Sections:

  • Line 5: Complete this line if your mailing address is different from your voter registration address (where you live). If you are homeless, you can provide a friend/relative’s address, P.O. Box, homeless shelter, etc as long as it is a valid mailing address.
  • Line 6: Complete this line if you need your ballot sent to a different address than your residential/mailing address (for example, if you will be traveling during the next election). If you include a temporary address, be sure to change it after the election.
  • Line 7: If you wish to vote in primary elections, you must either select a party affiliation (7a) or register as Unaffiliated (7b). In Colorado, voters can participate in primaries if they are registered as Democrat, Republican, or Unaffiliated (unless another party also holds a primary, which is rare).
  • Line 8: Only complete this if you are updating an existing voter registration record.
  • Line 10: While optional, it is highly recommended that you provide an email address. It helps us get in touch with you if there is ever an issue with your ballot, such as a missing signature. You will also receive important election updates and reminders (but we will never spam or share your email address).
