Healthy Kids and Adults


Boulder County Healthy Kids and Adults is a program to improve the community's health by linking all eligible children, adults, families and pregnant women in Boulder County to available benefits and health coverage options. Our mission is to ensure that every resident in Boulder County has health insurance coverage and access to health care.

What Boulder County Healthy Kids and Adults can do for you!

  • Enroll eligible pregnant women, adults, children, and families into Medicaid programs
  • Obtain documents needed to apply for CHP+ and Medicaid
  • Complete annual CHP+ and Medicaid renewals
  • Assist in applying for SNAP
  • Access other local agencies for services
  • Access preventive health-care services for their children
  • Link families to Connect for Health Colorado

Child Health Plan Plus (CHP+) is low-cost health insurance for Colorado's uninsured children and pregnant women. CHP+ is public health insurance for children and pregnant women who earn too much to qualify for Medicaid, but cannot afford private health insurance. Colorado Medicaid is public health insurance for families, children, pregnant women, adults, persons who are blind or persons with disabilities and the elderly for Colorado residents.

When you apply, you apply for both Medicaid and CHP+. The program you qualify for depends upon family size, income and the age of the applicant.

Did you know?

Many uninsured children, pregnant women and adults may qualify for free or low cost health insurance coverage. In order to qualify an applicant must:

  • Be a Colorado resident
  • Be a U.S. citizen, national or have legal permanent residency
  • Meet income guidelines (listed below)

What Services are Covered?

CHP+ and Medicaid are comprehensive health insurance programs and provide coverage for a wide range of services including:

  • Hospital care
  • Vision services
  • Immunizations
  • Dental care
  • Mental health care
  • Prescriptions
  • Other health services

What is the cost?

Medicaid is free to eligible enrollees. CHP+ may have a premium of $75 a year for 1 child and $105 a year for 2 or more children. If you are assessed a family premium for CHP+ and need help paying please call Healthy Kids and Adults 720-722-1454/720-515-1454.

What should I bring for my appointment?

  • Picture ID and/or school ID if age 16 or older
  • Passport or birth certificate
  • Proof of income - pay stub or letter from employer
  • Proof of legal residency if non-citizen
  • Social security number
