Vital Records: Ordering birth and death certificates

Order in Person
Receive a certificate within 10-15 minutes. Pay by cash, check, money order, or credit card (Visa or MasterCard only). ID required.

Order by Mail
Takes 2-10 days. Pay by check or money order. ID required.

Takes 2-10 days upon submission of request. Pay by credit card ($10.00 surcharge applies).

Fee for Birth Certificates

$17.75 for one copy (or for search when no record found); $10.00 for each additional copy of the same record when ordered at the same time. An additional $10.00 fee will be added to the order when using a credit card, except when using our walk-in service.

Fee for Death Certificates

$17.00 for one copy (or for search when no record found); $10.00 for each additional copy of the same record when ordered at the same time. An additional $10.00 fee will be added to the order when using a credit card, except when using our walk-in service.

All birth and death requests require identification. Identification can be delivered in person or photocopied and faxed/mailed.

Birth Certificates

  • BCPH can issue certified birth certificates for births occurring anywhere in Colorado from 1904 to present day.
  • Boulder County issues only certified copies of birth certificates. They are acceptable for passport, social security, employment, and personal identification purposes.
  • The person named on the record, members of the immediate family, legal representatives of those named above, and others demonstrating a direct and tangible interest in the record may request a copy.

Death Certificates

  • Death records that occurred in Boulder County are available for the years 1880 to present.
  • Certified copies of death certificates may be issued to: parents; grandparents; stepparents; siblings; spouse; adult children, stepchildren, or grandchildren of the deceased; legal representatives of any of the above; legal representatives of the deceased; probate researchers; genealogists representing family members with appropriate credentials; others who may demonstrate a direct and tangible interest when information is needed for determination or protection of a personal property right.
  • All death certificates are processed out of our Boulder office. You may request a death certificate from our Longmont office, but please realize that we may need two additional business days to process your request.

You may submit a request at one of our offices, by mail, or online.

Boulder County Public Health does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. VitalChek can be reached through its website, www.vitalchek.com. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.
