Ordering Certificates by Mail
To process your request by mail, you will need:

Photocopy of valid photo ID (of the person making the request). Either a
current driver's license or passport will do.

identification.

Check or money order.

Please make checks or money orders payable to: Boulder County
Public Health Vital Records.

Please
do not send cash.

Birth Certificate fee:
$17.75 for first copy (or the search of files when no record is
found; the searching fee is non-refundable and non-transferable).
$10 for each additional copy of the same record, ordered at the same
time.

Death Certificate fee:
$17.00 for first copy (or the search of files when no record is
found; the searching fee is non-refundable and non-transferable).
$10 for each additional copy of the same record, ordered at the same
time.
