Ordering Certificates in Person
To process a request for a birth or death certificate in person, you will need:
Valid photo ID (person making the request). Either a
current driver's license or passport is acceptable. Or, see the list of
additional
identification that is acceptable. Cash, check, or money order. -
Credit cards can be accepted only at our Boulder Office.
-
$17 for first copy (or the
search of files when no record is found; the searching fee is
non-refundable and non-transferable).
-
$10 for each additional copy of
the same record, ordered at the same time.
All death certificates are processed in our Boulder office.
You may request a death certificate from Longmont, but
please realize that it could take two additional business days to process
your request.
