Opening a Body Art Facility

Boulder County Public Health licenses and routinely inspects all body art facilities in the county. "Body art" facilities include any business that provides tattoo or piercing services.

To open a body art facility:

1. Complete the Boulder County Body Art Application  (PDF 42 KB) and Plan Review  (PDF 59 KB)
2. Bring application with a check (payable to "Boulder County Public Health") for $250 to:
