Operating a Body Art Facility
As a licensed operator of a body art facility, you are
responsible for protecting the health of your clients by abiding by all
state regulations. Please review the following information and resources,
as they will be helpful in complying with these regulations.
Licensing Application and Renewal
Boulder County Public Health (BCPH) must license all body
art
facilities before opening. All licenses must be renewed annually.

facility owners and operators. Providing this list does not constitute
a recommendation or endorsement of any of these businesses. If you
represent a business that provides one of these services and would

Exposure Incident Record (PDF)
Every time an operator at a body art facility comes in contact with
bodily fluids (vomit, blood, etc.) or has a needle stick or other
"sharps" incident an exposure incident report must be filed. It then
should be e-mailed
to cp@bouldercounty.org, or
faxed to 303-441-1468.
Hepatitis B Waiver

Boulder County Public Health strongly recommends that all body art
operators receive vaccination for hepatitis B, as they are at an
elevated risk. If an operator opts not to receive vaccination, they
must sign this waiver, and it must be stored onsite.
