Using the Boulder County Public Health
NFP Referral Form

• To fill out the form:
• Click inside the gray box to the right of Date.
• Type in the date.
• Use the TAB key to move to the next field, Last Name.
• Continue using the TAB key to move down through the form.
• To fill in a check box field, such as Spoken Language(s), either
  • Click inside the box with your mouse, or
  • Press the Enter key, or
  • Press the space bar.

• When you have finished the form,
• Scroll up to the top of the page.
• Print the form using the Print button, then fax to NFP at 303-413-7505.
• If you would like to keep an electronic copy of the form, click the Save button.
• Because of HIPAA concerns, please do not email the saved document to NFP.
