Entering a Colorado School
for the First Time
Day Care/Preschool & College
Students
New students must follow requirements for the
grade being entered as indicated below, AND they will need:
-
5 DTP/DTaP/DT/Td - type vaccines,
although they may enter with 4 doses if 4th dose was given at/after age
4. Students do not need "P" (Pertussis) if
they are 7-9 years
old.
-
4 polio doses, but they may enter with 3
doses if 3rd dose was given at/after age 4.
-
1 hepatitis B series (3 separate doses) -
required for all school grades.
-
2 MMRs (measles/mumps/rubella)
shots for all school grades.
-
2 varicella/chickenpox shots - 2
doses at least 28 days apart (preferably 3 months), or
disease history by health provider, or lab test for
kindergarten and grade 1 and 2.

1 varicella/chickenpox shot or
disease history in grades 3, 4, 5, 6, 7, 8, and 9 for
the 2009-2010 school year. Grades 10, 11, and 12 do not
require varicella for 2009-2010.

Colorado immunization requirements also exist for:
- Children in infancy through 4+ years old who attend
child care or preschools
- College students who attend state-supported schools
that have any on-campus residence housing
Contact your health care provider, local health
department, day care provider, or college health office for
more information regarding these requirements.
NOTE: Medical, religious, or personal exemptions to
Colorado immunization requirements are possible for school
and/or day care attendance, but a signature will be required from a
physician or parent on a Colorado "Certificate of
Immunization." (Forms are available from your health care
provider, your local health department, or from the
Immunization Program at the Colorado Department of
Public Health and Environment.)
