Reporting Assistance Program (RAP)

With rapid reporting, quick response, and investigation, and with appropriate control measures, we are dramatically reducing the spread of contagious disease and improving the overall health of the residents of Boulder County. 

The Reporting Assistance Program (RAP) is designed to give you the information and tools needed to report a communicable disease in a timely and efficient way. Components of RAP include:

• Conditions Reportable by all Physicians
  (PDF 95 KB): a list of all conditions reportable in Colorado (as defined by the Colorado Department of Public Health & Environment, or CDPHE).
• Procedure Flow Chart
  (PDF 67 KB): a step-by-step flow chart describing the procedure for reporting a disease (including fax, phone, and hotline numbers).
• RAP Sheet
  (PDF 332 KB): a form that can be photocopied and faxed, outlining all information required when reporting a condition to Boulder County Public Health.
• Disease Fact Sheets
  List of all communicable disease fact sheets available from Boulder County Public Health.
