Medicaid Eligibility
Identity and Citizenship Verification
-
The following documents establish BOTH U.S. citizenship
and identity. An individual needs to provide only ONE
of the following documents to meet the documentation
requirement:
-
U.S. Passport; or
-
Certificate of Naturalization; or
-
Certificate of U.S. Citizenship.
If the above passport or certificates are not available, an
individual must supply one document establishing identity
AND one document establishing citizenship.
Do not send copies of your documents through the mail.
Original documents must be viewed at a
Social Services office.
Any of the following documents establish an individual’s U.S.
citizenship:
-
A U.S. public birth record showing
birth in: one of the 50 U.S. states;
District of Columbia; American Samoa; Swain’s Island; Puerto Rico (if born on or after
January 13, 1941); U.S. Virgin Islands (if born on or
after January 17, 1917); Northern Mariana Islands (if born
on or after November 4, 1986); or Guam (if born on or after April 10,
1899).

Certificate of Report of Birth
(DS-1350).
-
Consular Report of Birth Abroad of
a U.S. Citizen (FS-240)/(FS-545).
-
U.S. Citizen I.D. Card (I-197 or
I-179).
-
American Indian Card (I-872).
-
Northern Mariana Card (I-873).
-
Final adoption decree with child's
name and U.S. place of birth.
-
Evidence of Civil Service
employment by the U.S. government before June 1, 1976.
-
Official Military Record of Service
(DD-214) showing U.S. place of birth.
-
Extract of hospital record on
hospital letterhead established at the time of the
person's birth and was created at least five years
before the initial application date.
-
Life or health or other insurance
record showing U.S. place of birth and created at least
five years before the initial application date.
-
Medical record that specifies U.S.
place of birth and was created at least five years
before the initial application date. Immunization
records are not acceptable.
-
Federal or state census record for
people born between 1900-1950 showing the person's age.
-
Institutional admission papers that
specifies U.S. place of birth and was created at least
five years before the initial application date.
Any of the following documents establish an individual’s
identity:
-
State-issued driver’s license,
current or expired, from any U.S. state.
-
I.D. card issued by federal, state,
or local government, with the same information as
provided on a driver’s license.
-
School I.D. card with a photograph
of the person.
-
U.S. military card or draft
record/military dependent I.D. card.
-
Native American Tribal document.
-
U.S. Coast Guard Merchant Mariner card.
For children under age 16 only, any of
the following documents establish identity:
- School record.
- School ID card with photograph of child.
- Day care or nursery school record.
- Written affidavit affirming the child's identity.
