I.D. For Me Program

This program was implemented to assist persons encountered by community members or law enforcement officers, who
due to dementia, disease, or trauma, is unable to advise of their name and/or current address.
The program assists in the identification of such persons by enrolling him/her at one of the participating
Boulder County law enforcement agencies. The contents of the enrolled person's database will be kept confidential,
with information being released only within the law enforcement and medical community as needed. At the time of
enrollment, forms with basic contact, personal information, and a liability waiver will be completed. The program
participant will also be photographed. They will be issued an I.D. bracelet with a unique number. Boulder County
Communications with maintain data on which law enforcement agency issued each bracelet. The various law enforcement
agencies will maintain the records of the residents to whom they have issued I.D. bracelets.
The identification system can be used by:
- Law Enforcement: Officers locating a person enrolled in the program can request a file check of the
enrolled person's record to obtain information on the person's family or caregiver.
- Citizens: Citizens locating a person who appears lost or disoriented can call the phone number located on
the bracelet and speak with a dispatcher. The dispatcher will ask for the I.D. number on the bracelet and contact the
proper law enforcement agency that will take action to return the person to their home.
- Caregivers/Family: Caregivers are provided with the phone number for their local law enforcement agency.
In the event a memory impaired person leaves the residence undetected, the caregiver can give the identification
number to the dispatch center so the pertinent information can be accessed, and the appropriate description relayed
to officers. A photograph can also be made available if needed.
Through community partnerships, including the Coal Creek Rotary, Boulder County Senior Service Agencies, Boulder
County Aging Services, and using funds donated by local law enforcement agencies, the bracelets and enrollment are
provided at no cost to citizens. This program will give officers the ability to handle contacts with disoriented
persons with care and expediency.
