The Colorado Department of Public Health and Environment (CDPHE) Communicable Disease Epidemiology Program requests clinical microbiology laboratories send certain culture isolates to the CDPHE Laboratory in addition to reporting positive results. Learn which isolates should be submitted for additional testing at CDPHE Laboratory.

There are several ways to report a case of disease to Boulder County Public Health.

Types of Cases

24-Hour Cases & Outbreaks

Report all confirmed and suspected 24-hour reportable conditions and/or outbreaks

  • During normal business hours, Monday-Friday 8:30 am – 5pm, call Boulder County Public Health: 303-413-7523
  • After hours, call 303-413-7517 for 24/7 on-call communicable disease assistance

All Routine Cases

Report all reportable conditions.

Colorado Electronic Disease Reporting System (CEDRS)

Infection Preventionists and laboratory workers are encouraged to use CEDRS for case reporting.
