How do I apply for CCAP?

We strongly recommend you consider attending an “OPEN HOUR” on Tuesdays and/or Thursdays from 11a-1pm at 3460 N. Broadway in Boulder or 1921 Corporate Center Circle, Suite 3F in Longmont. While these sessions are optional, they allow you an opportunity to meet with a CCAP caseworker, ask questions, get application or redetermination packet assistance and better understand the program. For best use of your time during the “Open Hours”, we strongly suggest you bring your completed your application and ALL the required documentations listed on the check-list. Please allow time for the caseworkers to explain the program, review how to pin and swipe the CCAP cards and explain the processing time. What documents do I need to submit in order to apply?

  • Your completed, signed and dated CCAP application
  • Your signed and dated client responsibilities agreement
  • All verifications listed on the client check list plus any additional forms (below) and documentation depending on your eligible activity.
  • Please note: We cannot accept photocopies of photo Ids or birth certificates. We must view the original. A county employee must verify that they viewed the original. DO NOT fax or scan copies of such with your application. If you have submitted verified copies of these documents to other programs such as Medicaid or food assistance, please note so on your application. We will accept previous verifications from other programs if they are still current and have not expired.

Please submit the last 3 months of paystubs for all adult members of your household (not including the current month) AND have your employer complete and submit a Verification of Employment (130 KB).

Very Important: If you have set, fixed hours every week: your employer should list those times in the grid and provide us with the number of hours you work each week, your rate of pay and the frequency of pay. We need this document signed by your employer, dated and submitted to CCAP immediately. Be sure your name and date of birth or social security number is on this form for easy referencing. If your hours vary from week to week, your employer will need to tell us “Earliest in/latest out”. We also need to know how many hours you typically work, your rate of pay and the frequency of pay. If you are a student:

  1. A letter from your school verifying the program you are in, that you are making satisfactory progress, the degree or certificate you will receive, and your anticipated graduation date.
  2. A copy of your school schedule, including days of the week and times of your classes. Be sure to also provide us with start and end dates of the quarter, semester, or session
  3. A copy of your current Financial Aid letter

How much will I have to pay?

The parent fee is based on your income, household size and number of children in CCAP care and is subject to change upon receiving prior notice from the county. The parent fee listed on your authorization will be due to the provider on the first day of each month. You must sign in and out at your provider AND use the CCAP swipe machine and your CCAP card to mark your CCAP child(ren)’s attendance.


Can I use my own Child Care provider?

Families enrolled in the Boulder County Child Care Assistance Program can choose their own provider as long as the selected provider has an open fiscal agreement with Boulder County CCAP. If you need help finding a CCAP provider please contact Children, Youth and Families at 303 441 3544 for English and 303 441 3564 for assistance in Spanish. They hold the most updated CCAP list and can provide you with the names of CCAP providers near your work, home or school according to the ages of your children and specific needs.


I have an existing case. How can I submit information to my caseworker?

Please note: We cannot accept photocopies of photo Ids or birth certificates via fax or email. A county employee must verify that they viewed the original. If you have submitted “verified copies “of these documents to other programs such as Medicaid or food assistance, please note so on your application. We will accept the verifications from other programs if they are still current and have not expired. Otherwise, please stop by either location with your original and have a county representative note on your photocopy that they have seen your original birth certificate(s) and/or photo ID(s).

What is the anticipated processing time?

By law, your caseworker has up to 10 days to process the change or verification, and up to 15 days to process a new application and/or redetermination. Submit your paperwork as soon as possible to avoid any gaps in care. Be sure you carefully review the check-list and submit every verification as requested. CCAP will not be authorized if your application is incomplete and/or if you are missing any of the required verifications.
