Food Assistance

General Information

Food Assistance (FA) or Supplemental Nutrition Assistance Program (SNAP), assists individuals or families with a portion of their monthly food costs. Food Assistance is issued to a Quest EBT Card which works like a debit card at a grocery store checkout counter or ATM and is kept secure with a private pin number.

Eligibility

Colorado PEAK is a State online service for Coloradoans to screen themselves and apply for medical, food, and cash assistance programs. Visit Colorado PEAK to see if you are eligible and to apply!

You may be eligible for Food Assistance if you:

  • Work for low wages
  • Are unemployed or work part-time
  • Receive TANF/Colorado Works or other public assistance
  • Are elderly or disabled and live on a small income
  • Are homeless

Applying For Benefits

There are many ways to apply for benefits. Select the option that is best for you.

  2. Print the English Application Spanish Solicitud or pick up a copy at one of our offices
  3. Email or Fax your application

  4. Come into one of our offices Monday, Tuesday, Thursday or Friday between 8:00AM-11AM and 1:00PM-3:00PM to be seen the same day for a walk in interview.

What To Bring When Applying


Proof Of Identity:

  • Driver’s License
  • ID Cards
  • Birth Certificate
  • Social Security number for everyone you are applying for. If you don’t have a number you will have to apply for one
  • Proof of immigration status for non-citizens who are applying for Food Assistance

Proof Of Address:

  • Lease Agreement
  • Mortgage Statement
  • Utility Bills (Electric, Gas, Water or Phone)
  • Rent Receipt

Proof Of Income

All applicants must provide proof of income before taxes or deductions in the form of:

  • Pay Stubs for the past two months
  • Wage Statement from employer
  • Benefit Letter from Social Security
  • Unemployment Compensation
  • Veteran’s Administration or pensions

Proof Of Assets

All cash and non-cash assets must be verified. If applicable, please bring:

  • Most recent savings and checking account statement
  • Statements showing value of stocks, bonds or investment accounts

Child Support Payments

If you make child support payments, and would like these payments to be considered in determining your income, please bring:

  • Support Order
  • Separation Agreement
  • Child Support Records

Medical Expenses

If you are 60 years old or older, or you receive federal disability benefits please bring the following documents:

  • Doctor or Hospital Bills
  • Prescription Receipts
  • Receipts for over-the-counter medical expenses if prescribed by a medical practitioner
  • Transportation costs to get to medical care

Child Care Expenses

Child care expenses may be considered in determining your benefit amount if you are working, looking for work, or are in training.
