Motor Vehicle Forms
A complete list of forms related to motor vehicle registration and titling is available on the Colorado Department of Revenue’s website. Some of the most frequently used forms are listed and linked below.
All forms must include an original signature and be returned to Boulder County Motor Vehicle Division, unless instructed otherwise. As noted below, some forms are only needed for mail-in requests. In addition, many services can be accessed online, eliminating the need for a form.

- Motor Vehicle Bill of Sale
- Power of Attorney Form – Motor Vehicle Only (DR2175)
- Disability Parking Privileges Application (DR 2219)
- Joint Tenancy with Rights of Survivorship (DR 2395, page 2)
- Branded Title Disclosure Statement (DR 2710)
- Manufactured Home Transfer Declaration
- Emissions Extension Application (DR 2376) (you can also use the online Emissions Extension Application)
- Military License Plate Application (DR 2002)
Mail-in Only
Only use these forms for mail-in requests. Also note that these services can be accessed online.
Least Common
- Statement of Fact (DR 2444)
- Application For Title (DR 2395) (usually submitted by dealership or lien holder)
- Verification of Vehicle Identification Number (DR 2698) (must be completed by an authorized VIN inspector)
- Lost or Stolen License Plate / Permit Affidavit (DR 2283)
- Farm Vehicle Registration Application (DR 2159)
