Child Health Plan Plus (CHP+)

Family Medicaid and Child Health Plan Plus (CHP+) are public health insurance programs for families, children, and pregnant women. Family Medicaid serves low-income families and pregnant women. CHP+ provides low-cost health and dental insurance for children and pregnant women who make too much to qualify for Medicaid. When you complete an application, you are screened for both programs and you are approved for one or the other based on the program’s income guidelines and other requirements.

The requirements for this group are the same as those listed for the Family Medicaid program. Other Medicaid programs make health coverage available to the elderly, and people with disabilities. Like private health insurance, both programs provide coverage for medical and dental care at participating providers. Medicaid and CHP+ both cover a range of services including:

  • Regular checkups and doctor visits
  • Immunizations (shots)
  • Prescriptions (medicine)
  • Prenatal care and labor and delivery
  • Hospital services
  • Mental health services
  • Eye care and eyeglasses
  • Hearing aids
  • Dental care for children (CHP+ has a $600 annual limit per child)

Depending on your household family size and income, you may need to pay an enrollment fee up to $105 once per year in addition to co-pays when you receive services. There are no monthly premiums and pregnant women have no co-pays.

Eligibility Requirements

Individuals who are eligible for ongoing health insurance benefits include:

  • Colorado residents
  • U.S. Citizens, legal permanent U.S. residents (for at least five years), asylees, or refugees. Other immigrants may qualify
  • Children under the age of 19, pregnant women, parents of Medicaid-eligible children, and adults ages 19-64

How to Apply

There are several ways to apply for benefits. Select the one option that is best for you:

  • Colorado PEAK is a state online service for Coloradoans to screen themselves and apply for medical, food, and cash assistance programs. Visit Colorado PEAK to see if you are eligible and to apply online.
  • Complete the Colorado Financial, Medical, and Food Assistance Application for Assistance (English), Solicitud (Espanol). Then Print the application and fax, email, or mail it to our office.
