The nurses from About the Nurse-Family Partnership Program (NFP) visit women in their homes during their first pregnancy and throughout the first two years of their children’s lives.

Individuals expecting their first baby or whose first child is less than one month old who meet income guidelines are eligible for the program.
Many working families qualify. Individuals who participate in other programs, such as food stamps, Medicaid, WIC (Women, Infants, and Children), and Temporary Assistance to Needy Families (TANF), may automatically be income-eligible.
If you are interested in NFP services, you are currently pregnant, and meet the above income guidelines, please contact our NFP coordinator at 303-413-7554.
To refer a pregnant woman, please complete a referral form and fax it to 303-413-7505.
